<p>There are many different types of therapists who could be called physical therapists, including physiotherapists, chiropractors, osteopaths, masseurs and some areas of occupational therapy. They all work to try and affect the joints, muscles, tendons and ligaments but have different philosophies and techniques to do this. However they do all have the same aim for patients with hip osteoarthritis and that is <b>to restore function by improving mobility and so relieving pain and stiffness.</b> This is all directed at the muscles, tendons and ligaments (the soft tissue) around the hip joint as there is nothing that can be done for the actual joint itself in hip osteoarthritis (unless you count a joint replacement!).</p>
<p>To do this a physical therapist will use different techniques, depending on their profession. Chiropractors, osteopaths and some physiotherapists use manipulation where they &#8216;crack&#8217; the hip, stretching the soft tissue around it and so improving mobility. </p>
<p>Many physiotherapists and occupational therapists give exercise programmes, including stretching to improve mobility and strengthening to improve the structure of the soft tissue around the hip and so support the joint better. This is vital to the long term stability of the hip osteoarthritis sufferer.</p>
<p>I have worked with many different types of therapists over the years and what type you go to and what works for you is very much up to the individual. At my clinic some hip osteoarthritis patients did better with a Chiropractor who &#8216;cracked&#8217; them and did massage, others with the physiotherapist who would use mobilization and massage and others were better with just the masseur who obviously used massage only. </p>
<p>All were given an exercise program no matter who they saw.</p>
<p><b>What sort of exercise program should you do for hip osteoarthritis?</b></p>
<p>This is what I recommend. You do need to do these three types of exercises.</p>
<p>1. Stretching- to increase the length of the muscles again and improve flexibility.</p>
<p>2. Isometric strengthening- these are exercises that strengthen the muscle but don&#8217;t move the joint and irritate your worn joint surface.</p>
<p>3. Isotonic strengthening- these are exercises that strengthen the muscle by moving the joint as you progress.</p>
<p>If you visit a therapist with hip osteoarthritis they should also do some massage as well or electrical therapy such as Interferential or TENS. Being a Chiropractor (Chiro means &#8216;by hand&#8217;) I still believe doing the work by hand is better. Why? Because a good therapist will be able to feel exactly which soft tissues are being affected and work exactly on the bad bit. It&#8217;s harder work for the therapist so is probably why a lot choose to use machines! (That was a bit bitchy wasn&#8217;t it?!)</p>
<p>How each exercise is done and how many repetitions you do will depend on the level of hip osteoarthritis you have and the type of therapist you visit.</p>
<p><b>N.B.</b> Do notbe tempted to just get a few exercises off You Tube or a catch all exercise DVD as the programme needs to be set for YOUR level of hip osteoarthritis and a programme should build up and then drop back to a maintenance level which most of those don&#8217;t.</p>
<p><b>Are there any problems with doing physical therapy for hip osteoarthritis</b><b>?</b></p>
<p>The main problem is that exercises are not a cure and must be done for life. Once you have your hip osteoarthritis reasonably controlled you can usually do them about three times per week to keep your condition maintained.</p>
<p>If I had a patient who moaned about that they usually got told that if they could not find 20 minutes three times a week then they can&#8217;t really be in much pain. All my programs could be done at home with no special equipment so there were no excuses!</p>
<p>Really there should be very little problems with a physical therapy program as long as you are doing a program that is designed for hip osteoarthritis sufferers and for your level of OA.</p>
<p>Anyway the long and the short of it is that if you have hip osteoarthritis you must do some type of physical therapy program to keep your joint moving and to strengthen the soft tissue in order to support your hip joint.</p>

 Obesity is a state of excess adipose tissue mass. It can also be defined as a progressive disease with excess fat accumulation which has multiple consequences. It is associated with the accumulation of adipose tissue in the body that is excessive for an individual’s height, weight, gender &amp; race to an extent that it produces adverse health outcomes.It is one of the most neglected yet visible risk factor contributing to the several other disorders worldwide. Obesity is the tip of iceberg which includes various diseases like cardio vascular disease, Type II Diabetes, osteoarthritis or even some types of cancers.On many occasions it becomes a cosmetic issue, but at the same time can cause various other risks &amp; decreased quality of life.

 Assessment of obesity-
</p>
<p>
 Body fat percentage is total body fat expressed as a percentage of total body weight. It is generally agreed that men withmore than 25% body fat &amp; women with more than 33% body fat are obese. A certain amount of fat is necessary for body to function properly as it is the key factor for storing energy, heat insulation, shock absorption &amp; other functions. It is stored in the form of adipose tissue in our body. Although not a direct measure of adiposity, the most widely used method to gauge obesity is the body mass index (BMI), which is equal to weight/height square (in kg/m square). The reference range used to categorize individuals is:

 Along with BMI certain other measures may also be taken into consideration to assess an individual in terms of nutrition. These are waist circumference, hip circumference, waist to hip ratio: a ratio of &gt;0.9 in women &amp; &gt;1.0 in men is considered abnormal, markers of nutrition e.g., mid arm circumference, skin fold thickness, etc.
</p>
<p>
 Two major patterns of obesity are observed in population, although these patterns are not gender specific, both the sexes can have any pattern, but it is seen that cross patterns are more dangerous. In females, the pattern seen is pear shaped obesity, i.e., they tend to collect fat in their thighs &amp; buttocks. In males, fat usually collect around the belly, giving them apple shaped pattern of obesity.

 ·For each individual, body weight is the result of a combination of genetic factor, metabolic, behavioural, environmental, cultural &amp; socioeconomic influences.

 ·Genetically- a person is influenced by how the body uses calories for energy &amp; how the body stores fat.

 But first, let&#8217;s talk about joint pain itself. The most common type of joint pain affecting over 20 million people in the United States is osteoarthritis (OA) pronounced (AH-stee-oh-ar-THREYE-tis). Osteoarthritis affects the hands, feet, spine, and large weight-bearing joints, such as the hips and knees. Simply put, osteoarthritis is the cartilage or cushioning between the bones wearing away in the joints. As it worsens, the cartilage disappears and bone rubs on bone. Sounds painful, doesn&#8217;t it?

 The onset of symptoms of osteoarthritis usually occurs between 30-40 years of age and by age 70, almost everyone has experienced symptoms. Generally before the age of 55, osteoarthritis occurs equally in both men and women. After 55, it becomes more common in women.

<p><strong>SYMPTOMS:</strong></p>
<p>
 · Chronic joint pain that gets worse when you become active after a period of inactivity
</p>

 · Over time, the pain persists even when you are resting
</p>
<p>
 · You can hear and feel grinding of the joint with motion
</p>
<p>
 · You feel increased pain during humid or damp weather
</p>
<p>
 · Your joints are swollen
</p>
<p>
 · You have lost some of your movement
</p>
<p>
 · You have experienced a loss of muscle strength around arthritic joints
</p>
<p><strong>CAUSES:</strong></p>
<p>
 The primary cause of osteoarthritis is aging. As we grow older, the water content of the cartilage increases, and the protein makeup of cartilage degenerates. Occasionally osteoarthritis develops in several family members, which also implies an underlying genetic or hereditary basis for this condition.
</p>
<p><strong>MEDICATIONS:</strong></p>
<p>
 Over-the-counter pain relievers can help with symptoms. Most doctors recommend acetaminophen (Tylenol) first, because it has fewer side effects than other drugs.
</p>
<p>
 If your pain continues, your doctor may recommend non-steroidal anti-inflammatory drugs (NSAIDs). These drugs help relieve pain and swelling. Types of NSAIDs include aspirin, ibuprofen, and naproxen.
